2026-2027 Lunch Program Information

Please consider filling out the Lunch Benefit Application for school year 2026-27.  This form is available to all families and if you qualify, it can benefit parents and students by providing free or reduced meals for the school year.  This form is also used as the basis for our school funding so it is very important for many reasons.  Information provided on this form is confidential and only accessible by the School Nutrition Director and Administration.

Meal Modification Information

Parents,

Thank you for the opportunity to provide lunches to your children daily. We are happy to know that you trust us with their nutrition and we do our best to meet the dietary guidelines put forth by the USDA while still making affordable, home-cooked meals.

For ANY child who will be eating school lunches that may have any form of medical allergy or intolerance needing modifications outside the meal pattern, the school will make modifications prescribed by a licensed medical authority (MD, DO, ARNP or PA only) to accommodate a disability.

Meal Modification Doctor Release Form 19B

For children that have issues with certain foods, I cannot keep them protected without proper documentation of the foods.

Although you may have a doctor's note on file in the office, the Lunch Director must have this specific form from the state in order to make changes to the meal pattern.

We are more than happy to make substitutions and meal modifications when it is medically necessary for your student's health.

For a student who does not have a disability, a request for meal modifications can be made and the school MAY choose to make substitutions but is not required to do so.

If your child has a medical form 19B on file, you do not need to have another filled out unless something has changed. If your child no longer requires meal modifications, please contact Dana Weninger to issue the release.
